Medly is a comprehensive application designed to assist patients in the self-management of various chronic conditions, including heart failure, diabetes, chronic obstructive pulmonary disease (COPD), hypertension, and mental health issues. It provides users with real-time insights into their health status, generating actionable feedback to help manage their conditions effectively from home. By serving as a communication bridge between patients and clinicians, Medly enhances clinical management through prompt monitoring and sharing of health information.

The app allows users to log essential data pertinent to their conditions, such as weight, blood pressure, heart rate, blood glucose levels, and symptoms. It is compatible with commercially available devices for automatic data capture and also facilitates manual entry of readings.

As a Class II Medical device, Medly requires a referral from a healthcare provider for access.
